Before I really knew what any of this would feel like long-term. Before everything became about tracking progress through lab results.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Then I started Lupron.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">What Treatment Is Doing (Without the Medical Jargon)<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Lupron lowers my testosterone. And the cancer feeds off testosterone.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So, the idea is simple: Take away what feeds the cancer.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">By February, my PSA dropped to <strong>0.8<\/strong>. That told me the treatment was doing something real. <\/p>\n\n\n\n<p class=\"wp-block-paragraph\">It&#8217;s also when I saw my testosterone for the first time: <strong>0.48<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Which means it\u2019s been pushed down to what they call &#8216;<strong>castrate level<\/strong>.&#8217; Not a term anyone loves hearing, but it means the drug is doing its job.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">Adding Xtandi<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In March, I started <strong>Xtandi<\/strong>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">If Lupron lowers testosterone, Xtandi makes it harder for anything left to even work.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So even if there\u2019s a small amount still in my body, the cancer can\u2019t really use it the same way.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">My Latest Numbers<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">When I saw my medical oncologist labs from yesterday posted to my hospital chart things got interesting. One I hadn&#8217;t expected them to post at all and two something seemed off. So, a little research was in order.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">April numbers:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>PSA: <strong>0.28<\/strong><\/li>\n\n\n\n<li>Testosterone: <strong>0.55<\/strong><\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">PSA kept going down but testosterone went up slightly.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And yeah\u2014that caught my attention.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">What That Actually Means<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">At first glance, it feels backwards. I expected everything to just keep dropping. But treatment doesn\u2019t always move in perfect straight lines.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">I\u2019m on a <strong>3-month Lupron cycle<\/strong>, and what I\u2019ve learned is toward the end of that cycle testosterone can creep up a little before the next injection.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">So that small shift from 0.48 \u2192 0.55 can just be timing.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">The Part That Matters More Than Anything<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">My PSA went from 9.19 \u2192 0.8 \u2192 0.28.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">That\u2019s the real story. Because PSA is what shows whether the cancer is responding. And right now, it is.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">What I\u2019m Paying Attention to Now<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">I\u2019m not ignoring the testosterone number. I\u2019m just not overreacting to one reading either.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">What matters is the pattern:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Does testosterone keep climbing, or was this just timing?<\/li>\n\n\n\n<li>Does PSA continue to drop or stay low?<\/li>\n\n\n\n<li>Do things stay consistent over the next few months?<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">This isn\u2019t about one lab result. It\u2019s about the trend.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">What Could Change Later<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">If my testosterone keeps rising, there are ways to adjust:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Shortening the time between Lupron shots<\/li>\n\n\n\n<li>Watching levels mid-cycle instead of right before injections<\/li>\n\n\n\n<li>Tweaking the approach depending on how everything lines up<\/li>\n<\/ul>\n\n\n\n<p class=\"wp-block-paragraph\">But none of that happens off one number.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">Where I Am Right Now<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">I feel OK where I&#8217;m at. Yet that doesn\u2019t mean I\u2019m ignoring reality. It just means I\u2019m not walking into every appointment expecting something to go wrong.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The treatment is working. The numbers show that.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And for now, that\u2019s enough.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">Life Outside the Labs<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">There\u2019s still life happening in between all of this.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">There\u2019s time for that trip to <strong><a href=\"https:\/\/rickollie.com\/es\/?s=boston\" target=\"_blank\" rel=\"noreferrer noopener\">Boston<\/a><\/strong> to be with Maya. And moments that don\u2019t revolve around appointments or results coming in May.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">And those moments matter just as much as anything printed on a lab report. Because I am not going to let cancer run my life.<\/p>\n\n\n\n<h2 class=\"wp-block-heading has-text-align-center\">What Comes Next<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">More labs injections and watching numbers. But now I understand what I\u2019m looking at.
